Hospice@Home
The Hospice@Home program supports clients who are living with a life-limiting illness in their wish to stay at home on their palliative and end-of-life care journey and provides respite for caregivers. This service is provided by a trained Personal Support Worker and service supervision is provided by a Hospice Care Coordinator.

This program supplements current PSW support in place from Home and Community Care Services.
PSW’s may support the client with:
  • Showering or bathing and assistance dressing
  • Toileting
  • Medication reminders
  • Assistance with safe transfers and mobility
  • Meal preparation
  • Social support
  • Assistance with routine, light household activities such as sweeping, dusting, dishwashing, vacuuming, laundry
  • Shopping and errands
Eligibility:
  • Client has PSW supports in place from Home & Community Care Services
  • A Matthews House Hospice volunteer is unable to meet the client’s care needs
  • Client has a life-limiting illness (PPS score is 50% or less)
  • Client requires assistance with Activities of Daily Living
  • Responsive or defensive behaviors (known to inflict physical harm to caregivers) must be managed before a PSW can provide services
  • Client is not currently living in a retirement home or congregate living setting
  • Client has a safe home environment with a clear pathway for PSW’s to access the home
  • Services for housekeeping tasks must be in the client’s personal living area
Frequency:
Matthews House can provide 4 hours/week (2hrs, twice/week and on occasion for caregiver relief, 4hrs, once/week). PSW support is available between 9:00am and 8:00pm, 7 days per week. Clients will be reassessed every 3 months to determine ongoing eligibility.
